Students must bring completed forms signed by a parent or guardian, including the medical history form filled out with the child’s name and birth date on each page as indicated.  The forms are available at your school or on JCHL’s website under Fairbury Clinic forms- https://www.jchealthandlife.org/locations/fairbury-clinic/

The price will be $104.00 for the physical and if the school requires a urine sample that will be $36.00 in addition to the physical price.  Urines are required for 7th grade and Kindergarten.  If a urine sample is required, please pick up a sterile urine cup ahead of time at the clinic and bring it with you to your appointment with your child’s name on it.  Collect the first-morning urine and then place it in the refrigerator until the appointment.

Your sports physical may qualify as a Well Child Check & Sports/School physical when you call to schedule, please let the scheduler know what insurance you have and when your child’s last Well Child Check was.  If you are not sure let us know and we can assist you in looking up this information.  729-3361

Mrs. Comer’s Class Visits Penpals

Mrs. Comer’s 4th graders paired up with residents from Cedarwood and were pen pals throughout the school year. On April 19th, they went to spend some time visiting with them and playing BINGO! They ended their visit with some tasty ice cream! Thank you so much to Ellie Livingston and the Jeffs Gone M.A.D. (Making A Difference) organization for setting up this partnership! It has truly been a great experience for the students and the residents!

Central Elementary Celebrates Earth Day

What better way to spend our afternoons than picking up trash to save our Earth?

On Wednesday, April 19th, Central Elementary students took time out of their day to learn more about our Earth with read-alouds, coloring pages, gifts and a trip to clean up our environment.

Each grade level took on their own area in Fairbury to clean! Preschool cleaned up Central and the playground, Kindergarten walked downtown to the square, first grade traveled to the city park and second grade walked to McNish Park! Thank you Central Elementary students for your act of kindness to help keep our Earth clean and safe!
